A bulbous nasal tip can be due to thick skin and an excess of fatty tissue vs. excess cartilage. The latter is most amenable to refinement by reshaping the cartilage. Removing excess fat has its limitations as it is usually replaced by scar tissue negating whatever refinements were made. These patients actually do the best by adding cartilage grafts to give a more sculpted look during a rhinoplasty.
