Juvederm: Juvederm is a smooth gel filler used to smooth out those wrinkles or folds that look like parentheses along the side of your nose and mouth. (nasolabial folds) Juvederm injectable gel is produced through Hylacross technology creating a high degree of cross-linking and promoting duration for up to 6 months or longer. A recent study showed that Juvederm can last up to one year.  Hyaluronic acid (HA) is a natural complex sugar found in all living cells, and one of the major components of the dermis connective tissue matrix, along with collagen and elastin.  HA retains water like a sponge, absorbing more than 1000 times its weight.  This helps to attract and maintain water, hydrating your skin and increasing its volume and density.  Dr. Rubinstein prefers Juvederm over Restylane for lip enhancement.  Results have otherwise been equivalent to Restylane/Perlane (except lower eyelids where Restylane/Perlane is superior).

Juvederm Potential Risks:  The most common potential side effect is bruising.  There are rare cases of allergic reaction at the injection sites manifested by itching and redness which resolves after a few months.

Juvederm What to expect:  There can be slightly more discomfort with Juvederm injection than with Botox.  Dr. Rubinstein will prescribe Betacaine plus, a potent topical numbing cream manufactured in Florida.  This cream is to be ordered through the office and then is shipped to your home in about 7 days.  Apply the numbing cream 30 to 45 minutes prior to your injection.  If you have not received your numbing cream in time, please arrive 30 minutes early for your appointment so that we can apply it for you. 

Injecting the lips can be more sensitive.  Occasionally, there is the need for numbing injections to further enhance your comfort.

 

After your Juvederm treatment:  Do not exercise the day of the treatment to minimize bruising. Continue to avoid Medications that are on our ?Medications to Avoid List? a few days after injection. Also continue to take Arnica if it was prescribed.  By continuing to avoid certain medications and taking your Arnica you will help heal any bruising that you endure.

Results are immediate.  There may be a little swelling for 2 to 3 days and mild bruising.  Schedule your injections so that you have a 2 to 3 day period off from work.  You will experience mild to moderate bruising which can be covered up with make up by the 2 to 3 rd day.  Results for VolumaLift will last 6 to 24 months (depending on which fillers were used).

Perlane & Restylane: Perlane & Restylane are cosmetic dermal fillers that replace volume and restore youthful contours to the skin to smooth away moderate to severe facial wrinkles and folds, such as the lines from your nose to the corners of your mouth (nasolabial folds). The hyaluronic acid in both products is made by the same biotechnical process.  The difference is in the gel particle size. Perlane gel particles are larger and designed for deeper injection than Restylane.  Restylane lasts about 6 months. Perlane lasts at least six months.  One recent study showed that Restylane has the ability to stimulate collagen production accounting for the long term results seen in some patients. Dr. Rubinstein finds that  Restylane and Perlane provide equivalent results when compared to Juvederm (except lips).

NEW! Restylane-L & Perlane-L : Restylane-L and Perlane-L are crystal clear injectable gels composed of hyaluronic acid, a natural substance that already exists in the body.  Restylane-L and Perlane-L contain 0.3% lidocaine.  The lidocaine in Restylane-L has been added to reduce the discomfort associated with the treatment.

Dysport: Dysport is a prescription injection for temporary improvement in the look of moderate to severe frown lines between the eyebrows (glabellar lines) in adults less than 65 years of age. 

Dysport Cost
:  First site $ 400, each additional site treated the same day is $250.

No Downtime:  With Dysport, there is little to no downtime after injection.

Visible results:  In clinical trials, the majority of patients (86% on average) had frown lines that improved from scores of "moderate" or "severe" to "none" or "mild", respectively, as assessed at maximum frown by a physician at the 30 day evaluation, representing at least a 1-grade improvement on a 4-grade scale where 0 was none and 4 was severe.  A 2+grade improvement was considered a treatment success.

Repeatable results: In clinical trials, the efficacy of Dysport was demonstrated with up to 4 repeated treatments.  Your results may vary.  You should wait at least 90 days between treatments.  The safety of Dysport has not been studied for more than 12 repeated treatments.

Worldwide Experience: Dysport is used for aesthetic purposes in 34 countries.  In U.S. clinical trials, Dysport safety was studied in more than 2400 aesthetic patients.  A total of 376 patients were studied for efficacy of the product.

Dysport History:  Though approved in the United States in April 2009, Dysport has a history of use in other countries since 1991.  First approved outside the U.S. for aesthetic use in 2001, Dysport has steadily gained in popularity around the world.  With its approval by the U.S. Food and Drug Administration, you, too, may join others worldwide who have been treated with Dysport.

 

Dysport Potential Side Effects:  The most common side effects are nose and throat irritation, headache, injection site skin reaction, upper respiratory tract infection, eyelid swelling, eyelid drooping, sinus inflammation, and nausea.  In some cases, Dsyport may cause serious side effects such as swallowing or breathing problems, that may be life threatening.
  
Dysport Follow Up:  A 2 week follow up after your first injection is optional but strongly recommended especially if this is your first treatment with Dr. Rubinstein. 

Dysport Vs. Botox:

- Dysport is made from the same botulinum toxin Type A as Botox and is injected in the same way as Botox. Both work by temporarily relaxing muscles, which reduces lines and furrows.

- There are claims that Dysport may work faster and last longer.  However, doctors who work with injectables have different opinions on whether Dysport is really more effective.

- Dsyport has fewer proteins surrounding the toxin and so should stimulate a weaker immune respnse to break down the toxin.  This may make it a good choice for Botox patients who find Botox no longer works because their body has developed antibodies.

Dysport is injected around expression lines -  This therapy is used to prevent and eliminate worry lines, frown lines, laugh lines, crow's feet, and other dynamic facial wrinkles (caused by over expression), on the upper third of the face (eyes, brows, forehead, etc).  It has also been used to effectively treat the platysmal bands in the neck and sometimes to treat over expression in the naso labial folds. 

- Dysport can improve your appearance and prevent the formation of deep expression lines that make you look older.  Dysport will not help fine lines or wrinkles that are not due to facial expressions.

- You can be treated with Botox and Dysport injections at both medical spas and dermatologist's offices.

- Dysport is said to be slightly cheaper than Botox.  Just having a competitor to Botox may bring prices down.  However, many physician including Dr. Rubinstein find that the reported savings is not necessarily valid because Dsyport has to be injected in more concentrated dose than recommended by Medicis, the maker of Dysport in order to achieve comparable results as with Botox.
 

 

 

 

 

Radiesse: Radiesse is made up of unique calcium-based microspheres suspended in a natural gel that is injected into the skin. Radiesse works with your body, below the skin?s surface.  Tiny calcium-based microspheres form a ?scaffold? to support and stimulate the growth of your own collagen.  Over time, the calcium-based microspheres gradually break down and are safely and naturally absorbed by your body.  Radiesse does more than just temporarily  fill-in facial folds and wrinkles ? it rebuilds your skin?s foundation and restores your youthful look. Results with Radiesse are immediate.  Radiesse typically lasts up to 12 months and is ideal for someone looking for longer lasting volume replacement than can be obtained with the HA fillers (Juvederm, Restylane, Perlane) and doesn?t want to come in for multiple treatments as required with Sculptra.   It is frequently combined with other fillers.  For example, Juvederm is better suited for lips, and Restylane and Perlane provide superior results for lower eyelid camouflage.  Radiesse will last longer and allow for more filling volume per syringe when injected into the cheeks, jaw line, and nasolabial folds when compared to Restylane and Juvederm.

How to Prolong the duration of your Radiesse results:  Results typically last twice as long as Restylane and Juvederm.  Radiesse can't be used in the lips or to treat fine lines.  The same concepts regarding Botox injections in areas of high motion and expression apply.  Minimize smoking and sun exposure.

 

 

 

 

 

 

 

Sculptra:  Sculptra is a synthetic injectable material known as "poly-L-lactic acid." Poly-L-lactic acid is biocompatible (a material that does not harm the body) and biodegradable (able to be broken down by the body). Poly-L-lactic acid has been widely used for many years in dissolvable stitches, soft tissue implants, and other types of implants.  Sculptra is injected below the surface of the skin in the area of fat loss. Sculptra provides a gradual increase in skin thickness and facial volume. Visible results appear within the first few treatment sessions.  Sculptra is ideal for treating volume loss because 1) lasts 2 years or longer) 2) more volume can be injected than Restylane or Juvederm at a lower cost (if averaged out over a two year period).  Radiesse is equivalent as a volumizer when compared to Radiesse.  One advantage over Radiesse is that it can be used to fill out lower eye lid bags.  Sculptra (like Radiesse) is often combined with other fillers such as Restylane or Juvederm for the more superficial lines and folds. Sculptra Cost:  One vial is $ 1100, two vials is $ 1900.  2 to 5 treatment sessions of 1 to 2 vials needed. 

Sculptra Potential Risks:  The most common side effects with the use of Sculptra include injection-related side effects at the site of the injection such as bleeding, tenderness or pain, redness, bruising, or swelling. These side effects generally last, on average, 3 to 14 days. One possible delayed side effect with Sculptra can be small bumps under the skin in the treated area. These small bumps may not be visible, and you may notice them only when you press on the treated skin. These bumps tend to happen within the first 6 to 12 months after the first treatment. Occasionally, these bumps go away on their own. Visible bumps, sometimes with redness or color change to the treated area, have also been reported. As with all procedures that involve an injection through the skin, there is a risk of infection.

Sculptra What to expect:  Immediately following a treatment session with Sculptra, redness, swelling, pain, bruising or all of these signs can happen in the treatment area. These signs usually go away in a few hours to a few days. Some have been known to last up to 14 days. Dr. Rubinstein will give you specific post-treatment care instructions. You should massage the treated area (at least 5 times each day, 5 minutes, 5 days) for several days after the treatment session. Within the first 24 hours after treatment, an ice pack should be applied for a few minutes at a time to the treatment area to help reduce swelling. Wrap the ice in a cloth and avoid putting ice directly on your skin. Avoid excessive sun and UV lamp exposure until any initial swelling and redness has resolved. Most patients feel comfortable going back to their normal activities following treatment. Make-up may be applied a few hours after treatment if no complications are present (for example, open wounds or bleeding).

After your Sculptra treatment:  Do not exercise the day of the treatment to minimize bruising. Continue to avoid Medications that are on our ?Medications to Avoid List? a few days after injection. Also continue to take Arnica if it was prescribed.  By continuing to avoid certain medications and taking your Arnica you will help heal any bruising that you endure.   If you have had Sculptra injections, massage the injected areas in a circular manner 5 times per day for minutes for days. Results from Sculptra are not immediate.  At your first treatment visit, it may appear that Sculptra worked immediately because of swelling from the injections and the water used to dilute Sculptra. In a few days, when the swelling goes down and the water is absorbed by your body, you may look as you did before your treatment. Sculptra takes time to gradually correct the volume loss in your skin. Dr. Rubinstein will decide the appropriate number of treatment sessions and the amount of Sculptra you will need at each session. Patients with severe facial fat loss may require 3 to 6 treatment sessions. Treatment results will differ for each person. In a clinical study, the treatment results lasted for up to 2 years after the first treatment session, in most patients. Touch-up treatments may be needed to maintain the desired effect.

Sculptra Follow Up:  Sculptra works by stimulating your body to produce its own collagen.  When injected, the filler is 99 % water and only 1 % active collagen stimulator.  There is an immediate effect.  The next day, the water component of the injection disappears leaving little to no visible results.  With each day, your body will begin producing its own natural collagen with maximal effect by 6 weeks.  You will look younger and younger each day that goes.  You will be ready for your next injection after 6 weeks.  On average, most people require 3 treatment sessions.  From the time the last treatment is completed, results will last 2 years and longer.  A boost injection will be beneficial in maintaining the results every 2 to 3 years as we continue to lose facial volume as we age.

How to Prolong the duration of your Sculptra results: Results will last at least 2 years.  The most recent study that was completed showed that at 2 years most patients showed persistence of their results.  Future studies will probably demonstrate that the Sculptra effects, like fat transfer injections, are long lasting but that additional boost injections may be needed to replace ongoing facial volume loss that comes with the aging process.

VolumaLift:  Deflation of our face results in sagging.  Replacing this lost volume can have a lifting effect.  VolumaLift entails injecting Restylane and or Juvederm( 6 to 8 syringes), Radiesse (3 to 5 syringes) or Sculptra (average 3 treatment sessions, 1 to 2 vials per treatment).  The injections are placed deeper than traditionally used for wrinkle and fold management into those areas deficient in volume.  Combinations of the above mentioned fillers are typically used.  The deeper injections also result in a longer duration of effect. 

VolumaLift Potential Risks:  The most common potential side effect is bruising.  There are rare cases of allergic reaction at the injection sites manifested by itching and redness which resolves after a few months.

 

VolumaLift What to expect: The procedure entails injecting multiple syringes of filler at time to replace lost facial volume.  For example, up to 8 syringes of Restylane is often injected at a time.  For your comfort, numbing cream and numbing injections are used to provide an almost pain free experience. How plump do you want them?

 

Full, voluptuous lips are stylish and a sign of youthfulness. Not only can plumper lips revitalize your smile, but also it will smooth unsightly lines around the lips. If you are looking for a quick and easy enhancement to your appearance—with a range of options to suit your budget—lip enhancement might be the right treatment for you.

 

There are three options to consider: injectable fillers, implants, and a surgical lift. Each has their advantages; the choice for you should be made with Dr. Rubinstein after careful evaluation and education regarding your options.

 

A small gauge needle injects the filler into the lip border. There is minimal pain, minor swelling, and no recovery time. There are little to no side effects, with results showing immediately. The length of time that the fillers remain depends on the substance used. Fillers are derived from collagen, hyaluronic acid, or synthetic material. Hyaluronic acid (Restylane® or Juvederm®) is biodegradable with results lasting six to nine months. 
 

Implants are surgically placed from the corners of the inside of the lips. There may be mild swelling for three to five days and the results are permanent. A synthetic implant, such as expanded-olytetrafluoroethylene (e-PTFE), has small pores throughout the material that allows for tissue ingrowth. The implant material becomes incorporated with the body in a natural way. Sensation and function of the lips are not affected by this procedure. There are also implants available that are derived from processed human tissue, such as Alloderm®. The results, however, are not permanent, diminishing after six to 12 months.

 

The lift lip surgically shortens the distance between the bottom of the nose and the top of the upper lip to give the appearance of greater fullness. An incision is made in the upper lip that follows the lip line. A section of skin is removed and the lip is lifted to create a fuller look with more pout.
