Therma-Frax Neck Lift

The revolutionary Therma-Frax Neck Lift procedure combines the best of both worlds- Thermage radiofrequency skin tightening with Fraxel laser skin resurfacing.  As we age, our face loses volume and our skin loses elasticity and tone.  To make matters worse, sun damage manifests itself progressively with wrinkles, age spots and spider veins.  A recent survey conducted by the AAFPRS showed  most patients are looking to avoid going under the knife.  Instead of more invasive face lifts, they are seeking out new minimally invasive procedures that tighten the skin and treat the sun damage without the protracted recovery seen with older lasers.

Non-surgical technology used to smooth out skin laxity has evolved over the years.  Thermage has been and remains the leader and gold standard since it was introduced in 2002. Unlike other copy cat technologies, Thermage provides even, safe, heating of the deeper facial structures. This creates corresponding progressive skin tightening by producing new collagen over a 2 to 6 month period.  The face is also contoured by decreasing excess fatty deposits in the neck and jowl areas.  Unlike some of the other inferior technologies, just one treatment is typically needed.  With the new Thermage NXT system, not only are results more dramatic but patient comfort has also dramatically improved.  Most patients do not require medication prior to treatment.

Skin resurfacing, where by wrinkles are erased and sun damage eliminated has been increasingly popular over the years, often replacing some of the older, deeper chemical peels.  Like the deep chemical peels, safety issues and prolonged downtime has continued to be a problem.  It isn’t unusual to lose skin color in lasered areas and to require 2 to 4 weeks of recovery.  Over the years new laser technology has proved to be much safer with minimal to no downtime.  Unfortunately, results have not been impressive with these non invasive laser therapies- with one exception, Fraxel.  When the Fraxel re:store laser was introduced over 5 years ago, it revolutionized the skin resurfacing industry.  By only treating a fraction of the skin at a time, much like a pixel on a camera, in a series of 3 to 5 treatments dramatic results can be  accomplished safely without the downtime.  Fraxel most recently introduced the latest fractionated laser, Fraxel re:pair.  This laser provides equivalent or better results when compared to the most aggressive traditional lasers, and is particular useful in treating stubborn deeper wrinkles and advanced sun damage. Fraxel re:pair also has a dramatic effect on skin tightening much like Thermage but with a different mechanism of action.  Downtime is also a matter of days compared to weeks with the older CO2 lasers.  Other laser companies have tried to copy Fraxel over the years and have fallen short on efficacy, safety and downtime.  Patients often ask for Fraxel by name.

Dr. Rubinstein is among the few that has both Thermage, Fraxel re:store and Fraxel re:pair on site.  He has found that the combination of Thermage’s radio wave induced skin tightening of the deeper facial structures and contouring with Fraxel’s more superficial skin resurfacing and tightening have created an exponential increase in skin tightening than if performed alone.  Combining one and one yields four.  Thermage is typically performed first followed by Fraxel.  For the most dramatic skin tightening, Fraxel re:pair is used.  For those patients that need less tightening and who’s wrinkles and sun damage is less advanced, Fraxel re:store is used. 

Despite these dramatic skin tightening results, Dr. Rubinstein, a facial plastic surgeon, cautions that results are not equivalent to a more traditional face lift.  For those patients who want to delay going under the knife, ThermaFrax is an ideal procedure.  On a final note, replacing lost facial volume with fillers i.e. Sculptra, can enhance and augment the results seen with skin tightening procedures.  Whether it’s with a face lift or Therma-Frax, a volume replacement procedure is a great enhancer.  Balanced rejuvenation provides the most natural results.

LASER HAIR REMOVAL 

The Most Effective Method of Removing Unwanted Hair

For both men and women, unwanted hair can be difficult to treat. Traditional treatments, such as electrolysis can be painful, less convenient, and can result in scarring. The hair follicles are destroyed by tediously inserting a needle electrode into each individual hair follicle. An electric shock then kills the hair follicle. Treatments often take years. In contrast, specialized lasers can destroy the same hair follicles over large surface areas in just minutes. The laser uses light energy scanned over a wide area. The light is selectively absorbed by the pigment in the hair follicle sparing the surrounding skin. It is this selectivity that allows for these treatments to be performed quickly, safely, and with minimal discomfort. Normal activities are resumed immediately.

Laser (and electrolysis) targets only those hair follicles, which are visible i.e. in the growth phase of the hair cycle. Only 25 % of hairs are in their growth phase at any one time. The growth cycle lasts about one month. It is for this reason that on average, 4 treatments, at one-month interval, are most effective. Those hair follicles which have been destroyed will not regrow hair. It is possible, with time, for dormant hair follicles to become active and grow new hair requiring touch up treatments.

Laser hair removal technology has advanced tremendously over the past few years. Until recently, a major limitation in laser technology was that people with darker skin could not have safe laser treatments. The laser light was absorbed equally by the hair follicle and darker surrounding skin because the lasers could differentiate between the brown hair pigment and the darker skin pigment. This created a high risk for burns and scarring. Due to this risk, people with tan or darker skin could not have safe laser hair removal treatments. Dr. Rubinstein uses the latest laser technology, the Long Pulse YAG laser, which is super selective for the hair follicle pigment allowing for safe treatments even in people with dark skin.

Another advance in hair removal therapy is the ability to permanently treat bothersome ingrown hairs. Men with irritated ingrown beard hairs can now have successful laser treatments to alleviate this condition. In just a few treatments, the laser decreases the density and thickness of the hair follicles making them less likely to become embedded under the skin.

The only remaining limitation in laser hair removal is in those people with white or very blond hair. The light hair does not contain sufficient pigment for laser absorption. Electrolysis and waxing, performed at the skin care center, remains the only option.

LEG SPIDER VEINS

Enjoy the look of legs without unsightly red, blue, and purple spider. Dr. Rubinstein and his staff offer the latest in spider vein treatments. Traditional laser treatments resulted in poor results for spider veins greater than 2 mm in size. Laser treatments were also contraindicated in people with darker skin. Dr. Rubinstein uses the new Long Pulse YAG laser to successfully eliminate spider veins as large as 4 mm safely in all skin types.

Prior to laser treatment, a comprehensive examination is performed to determine if there are any underlying venous problems. Often, spider and varicose veins develop from a problem with the valves in the deeper veins causing an increase of backpressure. Venous Doppler studies may be performed to test the valves in the deeper veins. If there is a deeper venous disorder, surgery may be needed prior to spider vein treatment to ensure long lasting elimination of the spider veins.

The treatments are performed in our relaxed spa environment There is minimal discomfort and routine activities are resumed immediately. Treatments last about 30 to 45 minutes. Two to three treatments are needed with 8-week intervals between treatments. Spider veins larger than 4 mm respond best to injection sclerotherapy. The smaller spider veins are then eliminated by laser treatments. After the treatment, there can be some mild short lasting tenderness at the treatment site. The treatments result in coagulation of blood within the veins, which disappear gradually over several weeks as they are absorbed by the body.

After the laser and sclerotherapy treatments special support hose available on site are worn for one week. Routine activities are resumed immediately. It is important to avoid hot baths and strenuous exercise for several days. Bruises may develop around the treated areas and usually disappear after the first week.

LASER FACIAL (Laser Toning)

For deep lines and scars, traditional surgical laser resurfacing is still the treatment of choice. Traditional skin rejuvenating treatments, such as CO2 and Erbium lasers, deeper chemical peels, and dermabrasion exfoliate the protective outer layer of skin.  However, these treatments require anesthesia, bandages and a 5 to 10 day recovery period. The Laser Facial, was designed to treat the signs of aging in a softer and gentler way by preserving the outer skin layer and selectively stimulating the dermal (collagen) layer.

Dr. Rubinstein’s unique Laser Facial has three components that address the three different skin layers in one 60-minute treatment. The first step utilizes microdermabrasion (or glycolic acid peel) to gently exfoliate only the dead outer (epidermal) layer leaving behind healthier smoother skin. This also allows for more effective laser penetration (2nd and 3rd steps) into the dermal or collagen layer stimulating new collagen formation. The use of two different lasers provides for superior results while protecting the outer layer of your skin. Each of the two laser penetrates the dermal layer (collagen layer) at a different depth allowing for complete rejuvenation, resulting in skin toning, eliminating fine lines, spider veins and age/sun spots. Typically, four 60-minute treatments are performed at one-month intervals. Final results are seen by 6 months. Rejuvenation is maintained with monthly microdermabrasion or glycolic peels or quarterly laser facials. The cost is only one third of traditional laser resurfacing and no anesthesia is needed.

Facial Spider Veins

Unsightly facial spider veins can now be treated safely and effectively by laser therapy. Some of the causes for facial spider veins include heredity, hormonal therapy, birth control, rosacea, sun exposure, and the natural aging process. Rosacea is a common skin condition, which results in embarrassing red facial flushing and spider veins. Dr. Rubinstein uses the latest in laser technology to treat spider veins as well as rosacea.

Traditional treatments include inserting a needle into the spider veins and using an electric cautery to burn the blood vessels. This frequently resulted in pain, blisters, frequent change in skin color and possible scarring. Other treatments included sclerotherapy injections. A solution is injected into each spider vein resulting in collapse of the blood vessel. This treatment is effective but very time consuming and inefficient. Sclerotherapy is still used for treating very large spider veins on the legs. The laser light is selectively absorbed by the spider veins. In most cases, this results in collapse of the blood vessel wall and immediate disappearance of the spider vein. In larger vessels, the blood coagulates and the body absorbs the vein until it finally disappears over a 4 to 6 week period. On average, two treatments are needed at 4-week intervals. Dr. Rubinstein uses different lasers for different types of facial veins. He uses a diode laser for very small pink/red spider veins. For larger purple facial spider veins, the long pulse YAG laser is used because it emits greater light energy penetrating deeper into the vein, making treatments more effective with minimal discomfort.

There is minimal discomfort with the laser treatments. The sensation is similar to that of a rubber band being snapped on the skin. Some patients elect to have topical anesthetic cream applied one hour before the treatment. Treatments last about 15 to 30 minutes. After the treatment, there is mild skin tenderness, redness and swelling which can last several hours. Occasionally, some blistering can occur. This can be covered with make up and will fade away in about one week. Vigorous exercise and excessive heat exposure should be avoided for 48 hours. Use of any harsh skin cleansers or skin products with vitamin C, fruit acids, or Retin A 48 hours before and after the treatment should be avoided. All other routine activities can be resumed immediately. Regular use of sunscreen will help prevent new spider veins from developing.

Stress has negative effects on our physical and mental well-being. The physical changes influence your appearance and cause premature wrinkling. Learn how stress affects you and what you can do to minimize its impact.

 

Stress aggravates many parts of the body, including the skin. Our body's reaction to stress is the "fight or flight" response. In order to prepare for action, the body produces higher levels of cortisol and adrenaline. The body does not distinguish between physical or emotional stress; blood pressure goes up and the body is put on high alert. When stress remains constant or becomes chronic, this causes the most damage. Do you remember waking up with a pimple on your big day—wedding day, job interview, major presentation—wondering why of all days? Your body has been reacting to the stress by producing higher levels of cortisol, which has caused changes in the skin allowing acne to flare up.

 

Science has proven that chronic stress leads to premature aging. Studies have looked at the DNA and chromosomes of people who experienced high levels of chronic stress. If DNA is like a strand of string, each strand has a protective tip called a telomere. These thin as we age and lose their protective telomeres. Participants with chronic stress had DNA that appeared much older than their actual age. This biological evidence seems to prove that stress contributes to premature aging.


As we age, the skin loses elasticity and becomes thinner. The skin begins to break down and wrinkles appear. The wrinkle eventually becomes a crease and the crease a furrow. An example of this is crow's feet.

 

You can learn to limit the affects of stress on your facial features. Start with your attitude; stay positive and be flexible. Studies have shown that people who are "go with the flow" and known as resilient, can manage stress better and seem to age better too. As the saying goes, it is not what happens to you, but how you deal with what happens to you that is important. Another tip is to become more aware of when you are furrowing or squinting. What is contributing to this? It may be due to worry, sunlight, poor vision, intense concentration, etc. If you are aware of when you are tensing these muscles, then you can learn to temper and adapt new habits, instead of contributing to premature wrinkling.

Meet Patricia

On April 17, 2009, Patricia had a fraxel re:pair laser treatment for acne scarring.  She also suffers from rosacea, a common chronic condition where patients develop flushing of the skin and facial spider veins.  Fraxel (middle photo) dramatically diminished her acne scarring and sun damage.  After her second of four mini laser facials, her rosacea already shows dramatic improvement.  Patricia is a good example of how combination skin therapies create optional outcomes.

Laser is an acronym for light amplification by the stimulated emission of radiation. It is a high-energy beam of light that can selectively transfer its energy into tissue. The wavelength peaks of the laser light, pulse durations, and how the target skin tissue absorbs this, determine the clinical applications of the laser types.

Over 40 years ago, the first lasers were used to treat skin conditions. The argon and carbon dioxide laser lightened birthmarks, but had a high rate of scar formation. Major advances in lasers over the past 20 years has revolutionized the use and treatment of many skin conditions. Today, there is a wide spectrum of laser and light technologies available for skin rejuvenation and resurfacing.

he skin is composed of the epidermis and the dermis. Superficial wrinkles and pigmentation problems generally originate in the epidermis. Within the dermis are two layers-the papillary dermis and the reticular dermis-both of which are constructed of collagen, long fibers that loosen and stretch with age and sun damage. If the deepest layer, the reticular layer, is damaged, scars result; while the upper layer of the dermis, the papillary layer, heals from injuries without scarring.


The extent of laser resurfacing depends on the type of laser used. It can diminish wrinkles, skin irregularities, and scars. It can improve the overall skin tone and texture of your skin. After resurfacing during the repair process, your skin rebuilds the tissue allowing a fresh new layer to become the skin surface. Your physician will discuss with you the optimal strategy for improving your appearance, including the type of laser and treatment to best meet your needs.


Ablative lasers remove the upper layers of the skin using the power and heat of the light in a targeted beam. Common lasers include carbon dioxide (CO2) or erbium. The skin is wounded; anesthesia is usually required and there is considerable downtime while the skin crusts, sheds the scabs, and fully heals. The major advantage to this invasive laser resurfacing is that the results are dramatic and long-lasting. Post-operatively, patients must stay out of the sun to avoid damaging the new skin.


Non-ablative lasers include diode, intense pulsed light (IPL), or pulsed dye. These lasers generate heat in the dermis layer to stimulate collagen production and improve the tone and texture of the skin. The upper layers of the skin remain intact, which results in minimal to no downtime and very little discomfort. This treatment may be used to treat acne, veins and discolorations, permanent hair removal, and wrinkles.


A new laser treatment is called fractional resurfacing. The laser treats a fraction of the skin by causing microscopic "wounds" within the targeted areas in the dermis layer. These targeted areas are treated intensively, while leaving the surrounding tissue unaffected. This allows the "wounds" to heal must faster versus if the entire area were affected. Through the healing process, new healthy skin emerges. The United States Food and Drug Administration (FDA) has approved this new fractionated laser therapy to treat acne scars, age spots/brown spots, melasma, and wrinkles around the eyes.

 

If you are considering laser surgery, keep in mind that many procedures can be performed with different lasers. The choice of laser will depend on the skin condition, size of the treatment area, skin type, ethnic background, and patient expectations.

 

 

Common problems improved by lasers

 

Acne. Lasers can be used to improve active acne and reduce the appearance of acne scars. Multiple treatments may be necessary with a non-ablative or Fraxel laser. The laser works by shrinking the sebaceous glands that feed acne bacteria and stimulating collagen production. An ablative laser can be used to remove the outer layers of the scar tissue and stimulate collagen to tighten and reduce the visibility of the scar.

 

Tattoos. A non-ablative laser can be used to target the colors of ink most common in tattoos. Blue and red inks are the easiest to remove. It may take between six to 12 treatments over a period of two months to see noticeable improvement.

 

Fine lines and wrinkles. Various lasers can be used to improve the appearance of lines and wrinkles. It will depend on your skin type and the extent of wrinkles. The treatment will promote the growth of new, smoother and supple skin. The non-ablative and Fraxel treatments are less invasive, but require repeated sessions. The ablative laser is more invasive and requires additional healing time.

 

Birthmarks. Port-wine stain birthmarks respond remarkably well to laser treatment. The abnormal blood vessels that cause these marks are reduced in size by the laser. This results in a lightening of the treated area. Skin growths, facial "spider veins," and warts also respond to laser surgery.

 

Pigmented lesions. In order to treat pigmented lesions or sun spots, a laser that absorbs black or brown pigment is used to destroy the unwanted cells, while leaving the healthy cells intact. Most situations take more than one laser treatment, but some respond to a single treatment.

 

Skin resurfacing removes the epidermis and penetrates into the papillary dermis, which responds by producing new collagen.
