Redness, Inflammation and Rosacea

Redness, inflammation and rosacea can be effectively treated with the Excel V+ and Genesis laser systems. Add-on therapies include Hydrafacials combined with the red LED light and Rozatrol Booster. Our providers will often recommend a combination approach to treatment including the use of medical grade topical skincare products in order to obtain optimal results. Inflammatory conditions like rosacea are chronic and will require maintenance treatments over time.

Age spots, Sunspots and Discoloration

Age spots, sunspots and discoloration can be treated with a variety of different lasers, including the Excel V+, Clear & Brilliant, Fraxel Dual and Fraxel Repair. Alternative treatments include chemical peels and Hydrafacials combined with the red LED light and Brightalive Booster. Our providers will often recommend a combination approach to treatment in order to obtain optimal results. The use of medical grade topical skincare products adds to treatment results, as does proper SPF usage and Heliocare oral supplementation.

Melasma

The pathogenesis of melasma is poorly understood but it is thought to result from genetic and hormonal influences in combination with ultraviolet (UV) radiation. Melasma is a chronic condition that must be managed and maintained over time, often requiring a combination approach to treatment. Minimizing UV exposure with sun avoidance, proper skin care and SPF usage, and Heliocare oral supplements are the foundation of melasma treatment. Our providers will place you on a customized skin care regimen designed to prepare and protect your skin in preparation for resurfacing procedures. The Clear & Brilliant Permea laser is a great option for lightening melasma, as are chemical peels. Both therapies are safe and effective on all skin types including skin of color.

Acne & Acne Scarring

There are several treatment options for patients experiencing active acne breakouts and those with post inflammatory hyperpigmentation (PIH) from acne that has healed but left a stubborn red or brownish mark. These options include the Isolaz, Excel V+ and Genesis laser systems, as well as chemical peels, HydraFacials, and a proper skincare regimen using high quality medical grade products. Some patients may require prescription medications in addition to their skin treatments.

Many patients with a history of moderate to severe acne are left with depressed acne scarring on their face and body. Depressed scarring can be treated with several different laser systems including the Fraxel Dual, Fraxel re:pair and Laser Genesis. Microneedling with PRP and Genius RF are non-laser treatment options that are equally as effective at stimulating collagen and improving the appearance of depressed scars.

In some cases, patients develop keloid scarring in response to severe acne. Keloids are raised, firm, often red scars that result from excessive collagen production in the skin. They can be asymptomatic, tender, or itchy and patients are frequently bothered by their cosmetic appearance. Treatment often includes a combination of laser therapy with the YAG Excel V+ laser and steroid/biologic injections to reduce inflammation.

Our providers will often recommend a multimodal approach to treating acne, PIH and acne scarring, combining several of these therapies to formulate a customized treatment plan for your skin care needs.

Wrinkles

There are many treatment modalities that stimulate collagen production to improve the appearance of wrinkles. Collagen stimulating lasers include Laser Genesis, Clear & Brilliant, Fraxel Dual and Fraxel Repair. Microneedling and Genius RF are also effective at treating wrinkles. Most patients require a combination approach to treatment, where collagen stimulating procedures are paired with injectables and sometimes surgery. Patients will benefit from muscle relaxing injections like Botox if their wrinkles are dynamic, meaning the result of muscle movement. Restoring lost volume with filler injections often improves the appearance of wrinkles by putting the skin on stretch. Depending on the degree of wrinkling and excess skin, some patients may require surgical intervention.

Skin Toning

Non-surgical skin toning is achieved through collagen stimulation. The Fraxel Dual and Fraxel Repair lasers can effectively tone problem areas, as can radiofrequency modalities like Genius RF. Patients who have significant skin laxity will often require surgical intervention to achieve optimal results.

Stretch Marks

Both color and texture must be considered when treating stretch marks. Fresh stretch marks are often red or purple in color, which benefit from treatment with a vascular laser like the Excel V+ laser. Stretch marks that are irregular in contour can be treated with the Fraxel Dual laser, microneedling or Genius RF. Most patients will require combination therapy that targets both color and texture. Early intervention is key, as immature stretch marks respond most favorably to treatment.

FAQs about Laser Treatments

What is the cost for laser treatments?

The cost for laser treatments varies depending on the laser system, area to be treated and number of sessions required. Our providers will work with you to devise a treatment plan that fits into your budget while still delivering excellent results. Patients generally find the cost of treatment to be very reasonable.

Are laser treatments painful?

Most patients do not find laser treatments to be very painful. There may be slight discomfort at times, but we use a combination of topical numbing and ProNox nitrous oxide gas to ensure our patients’ comfort during treatments.

Is there downtime for laser treatments?

The most common side effect is mild bruising. This can be prevented by avoiding the medications listed for you on our “Meds to Avoid list.” Dr. Rubinstein also uses a very small 31-gauge needle which is less likely to cause bruising. Eyelid droop, also known as ptosis, is rare in the hands of an experienced injector. This can be treated with Visine eye drops. Flu-like symptoms and headaches are also rarely reported.

Am I too young for Botox?

The downtime for laser treatments varies depending on the laser system, area to be treated, and treatment settings. Certain lasers have little to no downtime following treatment, while our more aggressive resurfacing lasers like Fraxel Repair will have about a week of recovery time.

Is there upkeep and maintenance with laser treatments?

Yes, all laser treatments will require upkeep over time in order to maintain results.

Who performs the laser treatments?

Unlike some Medspas, all laser treatments are performed by licensed providers including Physician Assistants, Nurse Practitioners, Registered Nurses and Medical Estheticians. All providers have been professionally trained on our laser systems and practice under the clinical oversight of Dr. Rubinstein. Dr. Rubinstein is a Fellow of the American Society of Laser Medicine & Surgery and a national trainer on minimally invasive treatments such as Botox and dermal fillers.

What is the difference between laser hair removal, electrolysis and waxing?

Laser hair removal (LHR) is a procedure where the pigment in hair follicles absorbs light emitted by a laser. This light is converted to heat which slows down or stops follicle growth. Treatments are often quicker and less painful than alternate hair removal options, as multiple hair follicles are treated simultaneously. LHR requires multiple treatments and often needs maintenance sessions over time. Electrolysis delivers short wave radiofrequency that damages hair follicles and disrupts growth. A probe is inserted into individual hair follicles, making the treatment more time consuming than LHR. Electrolysis requires multiple treatments and often needs maintenance sessions over time. Waxing involves the mechanical removal of the hair follicle, which requires indefinite maintenance as the hairs regrow.

What is the difference between lasers and microneedling?

Nonablative lasers (Excel V+, Clear & Brilliant, Fraxel Dual, LHR) create controlled thermal damage to the underlying dermis while sparing the epidermis of the skin. Certain wavelengths are used to target pigment (melanin) in the skin, treating sun damage and brown discoloration. Other wavelengths are used to heat the deeper layers, thereby stimulating collagen production and improving the appearance of texture and wrinkles over the course of several months.
Fractionated ablative lasers (Fraxel Repair) remove a portion of the outer layer of skin (epidermis) while heating the underlying tissue (dermis). This stimulates collagen production to improve the appearance of texture, wrinkles, and scars. As the epidermis heals, the skin often has a tighter appearance. This type of laser has more downtime than nonablative options but requires fewer treatments and generally yields greater results.
Microneedling also stimulates collagen production but does so by creating thousands of controlled “micro” injuries to the skin. RF Microneedling (Genius RF) creates the same minute injuries but additionally delivers radiofrequency into the deeper tissue to further enhance collagen production and remodeling.

What is the difference between lasers and chemical peels?

Nonablative lasers (Excel V+, Clear & Brilliant, Fraxel Dual, LHR) create controlled thermal damage to the underlying dermis while sparing the epidermis of the skin. They are great for treating issues with pigment (brown and/or red), excess dark hair, texture, scarring, and wrinkles. Fractionated ablative lasers (Fraxel Repair) remove a portion of the outer layer of skin (epidermis) while heating the underlying tissue (dermis). This type of laser is preferred for aggressive resurfacing of wrinkles and scarring.
Chemical peels use a variety of peeling agents to exfoliate the outer layers of the skin’s surface over the course of several days. Chemical peels come in a variety of types and strengths and generally the “deeper” or more aggressive the peel, the greater the result. Superficial chemical peels are great for treating acne, mild textural concerns, and issues with pigment (sun damage, melasma, post-inflammatory hyperpigmentation). Medium-depth chemical peels treat pigment but also stimulate collagen production by penetrating into the dermis. This deeper penetration improves the appearance of texture, wrinkles and scarring. Medium-depth peels will have greater downtime and often require the use of prescription medications to prevent infection while healing.
