LATISSE Discount for BOTOX & JUVEDERM XC patients - Save up to $100!
When you purchase a kit of Latisse - you can save up to $100 on your Botox and Juvederm XC treatment!
Here is how it works:
1. Gather your latest LATISSE receipt and bar code. (currently not a LATISSE patient? To be eligible for this rebate, ask your doctor if it's right for you and if so, purchase a kit between April 1 and July 15, 2010.
2. Ask Dr. Rubinstein if BOTOX Cosmetic and JUVEDERM XC injectable gel are right for you. If only one of these treatments is right for you, SAVE $50 on either BOTOX Cosmetic or JUVEDERM XC.
3. Receive treatments by July 15, 2010.
4. Have Dr. Rubinstein sign form given to you in the office and have the office staff provide you with the following:
a) An original itemized receipt for BOTOX Cosmetic and JUVEDERM XC
b) For BOTOX Cosmetic : The vial lot number, which Dr. Rubinstein's staff must write on the attached form.
c) For JUVEDERM XC injectable gel: The syringe container lid(s) (which you must mail with the attached form)
5. Take your rebate form home (given to you at Dr. Rubinstein's office) and go to http://www.latisse1year.com to register for the program. Fill out your information and you'll receive a confirmation printout with a unique promo code to mail with your rebate.
6. Use the form and mail with the following items:
a) Original LATISSE receipt and bar code
b) Original BOTOX Cosmetic and JUVEDERM XC receipt
c) JUVEDERM XC syringe container lid(s)
d) Confirmation printout from your online registration
