Combining different laser treatments is often necessary because one laser can not fix all problem areas. This is known as combination therapy.
My Facelift experience ... After four rounds of recurring oral cancer surgeries, I started receiving encouraging reports from my oncological team at Dana-Farber Cancer Center and Harvard Medical School in Boston, MA. I began, after nearly three years, to think of the future rather than follow a one-day-at-a time approach to life. In short, I began to plan again.
It took time, of course, for me to heal after the fourth extensive surgery--i.e., nerves and blood vessels needed to reattach and/or heal. After two years, I decided, with the consent of oral cancer team, to seek facial work. After a lot of research (I built a career as a journalist for a major news magazine.), I discovered a talented young cosmetic surgeon near my home named Dr. Ran Y. Rubinstein. Dr. Rubinstein, in my opinion, possessed outstanding credentials, including the distinction of holding the position of professor at New York Presbyterian Hospital, an outstanding cosmetic treatment center. He trains future cosmetic surgeons at the hospital as well as training surgeons in new techniques in the upstate area.
When I consulted with Dr. Rubinstein, I discovered a positive, upbeat person with knowledge of oral cancer. I later learned from my oncological surgeon that many plastic surgeons are reluctant to operate on oral cancer patients because of the amount of fat and tissue removed from the neck, which increases the possibility of facial nerve complications. But Dr. Rubinstein, as he put it, "liked the challenge" and felt more than capable of performing the work.
Dr. Rubinstein works carefully and in stages. In the build- up to surgery, he used several pre-operation procedures. He injected Sculptra over a period of months to fill in the hollows in my face, especially around the eyes and in my cheeks. Closer to surgery, he used Juvederm to add volume to my mouth and Botox to relax muscles around my eyes and mouth and to enhance surgery.
During surgery on my neck and lower jaw line area, Dr. Rubinstein ran into a tangled facial nerve in the portion of neck/face where the cancer surgeries had taken place. He worked meticulously for about 7.5 hours to complete his work. After the swelling went down, I was delighted with the operation. Not only had Dr. Rubinstein taken years off my appearance, he had moved the cancer scars toward my jaw line so they were far less noticeable. The "sagging" from removal of my lymph nodes disappeared. I couldn't have been happier. He received kudos from my oncological surgeon for the job as well.)
In the months following the surgery, Dr. Rubinstein closely followed the healing process. He suggested a serious of laser facial treatments, which lessened sun damage and obvious blood vessels on my face. As a plus, he later approved laser work on the neck area where all the cancer surgeries were performed. The redness--i.e., surface blood vessels--lessened.
Would I recommend Dr. Rubinstein and his talented, professional staff? I'll answer that question with an enthusiastic "Yes."
On December 9 of this year (2009), I celebrate the one-year anniversary of my facial surgery. I also see my oncological team on that day for hopefully another positive, cancer-free report. And, yes, life is good. The woman in the mirror looks less like a cancer patient and more like a happy, healthy individual--someone who, as a former high-altitude mountaineer, can tackle new adventures. Thank you Dr. Rubinstein for adding to my confidence.
On January 17, 2008 my face changed. My husband was in the hospital with a near death event from congestive heart failure. I woke up one morning while he was in the hospital and blood was streaming down my face. went to the first skin cancer doctor I could think of, and what a mistake! There I was diagnosed with basal cell carcinoma infiltrated. I went on to have mohs surgery for this diagnosis 5 times. I was badly scarred after my mohs surgery - to the point no one, including myself, could even look at me!

Memoirs of a Fraxel by Patty (featured in June 2009 Newsletter) I have been a patient of Dr. Rubinstein's for several years. When I received the e-mail that the latest and greatest technology had arrived, I wandered to his office to discuss just what the Fraxel laser could mean for me.
My skin quality was always something I had planned to address. I am fair with freckles and I don't tan, I burn. I must admit the years of sun damage and facial flaws were getting to me. Dr Rubinstein said that I was a perfect candidate for Fraxel re:pair.
I arrived at 8:30am on Friday, I was nervous but Dr. Rubinstein's staff greeted me and kept me comfortable while an anesthetic gel was applied to my face and neck. Dr. Rubinstein took a conservative approach to pain management and as soon as I received a few shots of Novocain, we were ready to go. It felt like he rolled over my face with the laser, making methodical sweeps until his pattern was complete. The laser felt like it was hot but it was not an unpleasant experience as I realized layers of age were being eliminated. The most sensitive areas were along the hairline and the chin line, but it was a passing discomfort as Dr. Rubinstein moved on to the next area. The procedure was over rather quickly and I was home before noon. Then I started to feel exhilarated! Day 1 - After The Procedure: Soon after getting home my face began to ooze and was quite messy. The frequent vinegar washes were very soothing as was the cream that I was constantly applying. By 3:00PM I was experiencing chills and felt a bit feverish, not unlike flu symptoms. None of this was alarming thanks to the thorough education I was given by Dr. Rubinstein and his staff. I knew exactly what to expect and when to expect it. Day 2 - Some Swelling: When I awoke on day 2 I was greeted with some significant swelling which subsided throughout the day. No pain medications were necessary Day 3 - Seeing Some Results: On day 3 the dryness woke me up and I had to carry the cream around with me so I could apply it frequently. The swelling had lessened but there was still a lot of redness. Day 4 - Here We Go: Day 4 was my check in day with Dr. Rubinstein. My skin had a cloth-like texture to the touch. There was still a little swelling, some redness, and dryness but now I could see some real improvement. Day 5 - Looking Better: On day 5 I let the shower run on my face. Using just my hands to clean my face, many layers of skin sloughed off. I may have stayed under a bit too long because my face felt rather sensitive. Day 6 - Starting to Relax: On day 6 I was feeling much more confident. I still had a lot of redness but the skin tone was starting to even out. Day 7 - I'm Liking This: Day 7 brought remarkable progress. The skin is still sensitive and pink in color but now it feels soft and supple to the touch. Day 8 - WOW!: By day 8 I felt great. I'm careful to apply cream and sunscreen - even while indoors. During my visit today Dr. Rubinstein reiterated that I could have pink tones and improvement for up to 3 months while my skin was in the process of rejuvenating - I'm Excited!

I always had dark circles and bags under my eyes even when well rested and they were getting worse with age. In addition, I always wanted fuller lips. As an esthetician working for a plastic surgeon, I was amazed at how common of a problem this was. Dr. Rubinstein takes the time to explain to patients the ageing process and how we all begin to lose volume as we age. Just like many of our patients, I had lost volume below my eyes and it was giving me a tired appearance. To correct this, he injected Juvederm under my eyes to fill in the hollows and camouflage the bags. He also filled my lips naturally with Juvederm. I had Botox injected into my forehead and glabella because I was developing deep worry and frown lines. Like most of our patients, I experienced very minimal discomfort during the injections and no bruising afterwards. I have also been using Latisse for the past few months. I never thought I could have lashes this long! I couldn't be happier with the way my eyes look! I love all the compliments I keep receiving.
